NYS DMV CDL Requirements

To renew your CDL.

Verify that your address is current on DMV records.
Provide a Medical Examiner’s Certificate if you choose Non-Excepted Interstate or Non-Excepted Intrastate types of driving.

Since the deadline for mailing in your Self Certification Letter was January 30, 2014, drivers who were not compliant in time or did not receive a Self Certification Letter may receive a Downgrade letter at this time.  It is important to respond immediately to avoid losing your CDL.

Remeber that the photocopied USDOT Medical Examiner’s Certificate should be legible, complete and on one side of a standard 8½” x 11″ sheet of paper and should be mailed before the expiration date of your current certificate, if renewing.

Also, if you have a medical variance on your license for diabetes, hearing, seizures, vision or a SPE certificate, you should make sure that your variance is current with the FMCSA.

Mail your certificate to:

Medical Certification Unit
PO Box 2601
Albany, NY 12228-0601
